Background Check Packet
Please download and print the form PDF by clicking the button above, fill out the forms completely, and return to the church offices during the week (Monday through Friday, 8:30am-4:30pm). The church offices are located across the street from the front entrance of the church – 4500 West Commercial Drive (the receptionist area is downstairs on the east side of the building).
Please Note:
- Please do not make any alterations using white-out or by crossing out information to make corrections. Unfortunately, this causes the forms to get sent back to us unapproved.
- The last page (Authorization For Release of Confidential Information) needs to be notarized – do not sign that form until you are with the Notary Public so that they can witness your signature.
